This site contains the following forms: PDF fillable/savable (ITAOP method), PDF fillable/savable (FBF method), PDF fillable/nonsavable, PDF nonfillable/nonsavable and non-PDF. Some forms are presented in different versions. Most of the PDF forms can be viewed only in Adobe Reader, version 6.0 or higher; some of the forms can be viewed in Reader, ver. 5 or higher. For some (older versions) of PDF forms, some of the features may not be available for Reader versions 8 and 9. You can submit your forms for conversion to make all features available for Reader ver. 8 and 9. See examples and demos of converted forms. Adobe Reader and Adobe Acrobat are different programs. Please open ITAOP and FBF forms only in Adobe Reader ver. 6 or higher. Do not try to view these forms in Adobe Acrobat. We included a protection code to prevent such view (the Acrobat full program can bring a damage to forms). The latest version of Adobe Reader can be downloaded from Adobe Reader download page.
